Patient App

The Patient App is how members access their DPC program. Branded to your organization, available in English and Spanish. Members book care, join video visits, manage prescriptions, view lab results, and follow care plans from their phone.

How It Works

The member journey from first login through ongoing care:

  1. Receive invitation — Once added to a roster, members receive an invitation to set up their account and a digital membership card they can store on their phone
  2. Passwordless setup — Members verify via email or SMS code — no passwords to manage. Guided onboarding confirms personal details
  3. Choose care type — Select from on-demand virtual visits, scheduled video appointments, in-person visits at a nearby facility, or async care for supported conditions
  4. Smart recommendations — Care suggestions based on eligibility, symptoms, and preferences guide members to the right care type — including async when the condition qualifies
  5. Book a visit or start async intake — For live visits, browse available providers with preferences for delivery mode, date/time, and location. For async care, complete a clinically validated triage questionnaire — a provider reviews the intake and determines next steps (prescription, callback, or escalation to a live visit)
  6. Post-visit care — Care plans with goals and tasks, prescriptions sent to preferred pharmacy, lab orders, follow-up scheduling, and condition-specific patient education appear in the app
  7. Chronic condition monitoring — For patients in care programs, ongoing tracking and check-ins with progress monitoring

App Screens

Home Dashboard

The home dashboard shows personalized cards — upcoming appointments, care plan tasks, medication reminders, and health program progress. Cards adapt to each patient's active care. Patients with connected accounts (family members, dependents) can switch profiles.

Get Care

The Get Care experience guides members through:

  1. Chief concern — Describe symptoms or select from common reasons
  2. Smart recommendations — Suggestions for care type (virtual vs. in-person, on-demand vs. scheduled, or async) based on eligibility and symptoms
  3. Provider selection — For live visits, browse available providers with preferences for delivery mode, date/time, and location
  4. Booking confirmation — Review details and confirm booking

For qualifying conditions, members can skip the visit entirely through async care. A clinically validated triage questionnaire captures the member's symptoms and history. A provider reviews the intake asynchronously and determines care — a prescription sent to their pharmacy, a callback, or escalation to a live visit.

Async care is available for:

  • Acute conditions — UTI, cold/flu/COVID, sinus infection, stomach bug, food poisoning, migraine, pink eye, seasonal allergies
  • Specialty conditions — Erectile dysfunction, HSV, hair loss, birth control, smoking cessation, vaginal yeast infection

Find nearby care locations with real-time availability, distance, contact information, and direct booking.

Primary Care

For patients enrolled in care programs, the Primary Care view is where ongoing care happens between visits:

  • Active care plans with goals and progress tracking
  • Biometric tracking — Log weight, blood pressure, and other measurements over time
  • Work items — Complete assigned questionnaires, acknowledge lab orders, schedule follow-ups
  • Async care tasks — Guided workflows for medication confirmations, health check-ins, and care plan tasks that patients complete on their own time without scheduling an appointment
  • Appointment timeline — Upcoming and completed visits linked to the care program

Health Programs

Patients can enroll in structured health programs (e.g., weight management with prescription medications). Programs include:

  • Enrollment workflow with eligibility verification
  • Monthly check-ins with health questionnaires
  • Prescription management and pharmacy coordination
  • Progress tracking with provider oversight

Medical Records

Complete access to personal health records:

  • Medications — Active prescriptions with dosing and pharmacy info
  • Lab results — Values with reference ranges and historical trends
  • Documents — Clinical documents, visit summaries, and imported records
  • Medical history — Diagnoses, allergies, and immunizations
